27. The Ministry of Culture of the Russian Federation, within 5 working days from the date of registration of the application for early termination of the open list, sends a written notice of early termination of the open list:

a) the holder of an open sheet;

b) the employer;

c) the customer (if any);

d) Russian Academy of Sciences;

e) to the territorial body of the Ministry of Culture of the Russian Federation or the regional body for the protection of cultural heritage objects that initiated the early termination of the open list in accordance with subparagraph "c" of paragraph 26 of these Rules.

28. In case of early termination of the open sheet in the event of work being carried out on the grounds specified in subparagraphs "d" - "e" of paragraph 5 of these Rules, the employer is obliged to ensure that a new open sheet is received by another individual who is in an employment relationship with him. Charging an additional fee from the customer (if any), not provided for by a previously concluded contract or offer, in this case is not allowed.

29. An open sheet, on the basis of which archaeological field work was not carried out, is subject to mandatory return to the Ministry of Culture of the Russian Federation within a period not exceeding 1 month from the date of expiration of its validity.

The Ministry of Culture of the Russian Federation notifies the Russian Academy of Sciences and the regional body for the protection of cultural heritage objects on the territory of whose activities the archaeological field work was supposed to be carried out about the fact of receiving an open sheet, on the basis of which archaeological field work was not carried out.

30. The public service for the issuance, suspension and termination of open sheets at all stages, including the preparation and submission of a conclusion by the Russian Academy of Sciences, is free of charge.

2. Place of residence (registration), contact phone number, e-mail address (if available).

3. Postal address to which the open sheet should be sent (the result of the consideration of the application).

4. Position, place of work, including:

full name of the employer;

location of the employer;

actual address of the employer;

contact phone number of the employer;

taxpayer identification number;

main state registration number.

5. Information about education (indicating the name of the educational organization, the terms of study, the direction of training (specialty), the qualifications awarded, the details of the document on education).

6. Territory for carrying out work to identify and study objects of archaeological heritage, including work aimed at searching for and seizing archaeological objects, with reference to administrative-territorial units (filled in in accordance with the documents substantiating the performance of the declared work).

7. The official name of the object of archaeological heritage where the applicant intends to carry out the declared work (filled in in accordance with the data of the unified state register of objects of cultural heritage (monuments of history and culture) of the peoples of the Russian Federation (indicating the registration number of objects of cultural heritage of federal significance) or a list of identified archaeological sites).

8. A list of documents justifying the work to identify and study objects of archaeological heritage, including work aimed at searching for and seizing archaeological objects, indicating the timing of their implementation in accordance with these documents.

I consent to the processing of personal data provided by me in accordance with the legislation of the Russian Federation.

The requirements of Articles 45 1 and 45 2 of the Federal Law "On objects of cultural heritage (monuments of history and culture) of the peoples of the Russian Federation" are known to me, understandable, and I undertake to fulfill them in good faith.
